First you must select the evaluation date then click on New. For example, to evaluate the current level of ambulation, select the daily activity by using the combobox or searching string (such as amb), then click on Walk Normally, and select the activity level the patient can do. 

PhysioGraphic allows you registering all type of activities. On the desktop version, it is possible to print the evolution of daily activities.

When over, click on the Ok button.
